| type | string | "Past", "Family", "Social", "Habits", "Diet", "Exercise", "Immunization", "Legal", "Consultation", "Health Maintenance", "Past Surgical", "Cognitive Status", "Functional Status" | The type of the history |
| rank | int | | The rank (or sequence) of the history |
| text | string | | The value (or text) of the history |
